Use this form to make changes or additions to your board order. We will respond to your request with a commitment to make the change and a revised cost quote if necessary. Changes must be communicated before we start production on the board. Once production starts we will not accept changes.

Please be as specific as you can be about the changes you want to make. We print this form and attach it to your current board order. We will confirm your changes and revise the quote as necessary. All change requests must be made before the board begins production. Verbal or other requests that are not documented with a change order are not guaranteed.
